This is the message Marie Larose, executive director of the organization, outlined when she spoke to members of the Collingwood General and Marine Hospital board on Tuesday.
The group is comprised of 16 health care professionals including specialists, nurses and registered nurses.
In the last year the group has started nine different programs including cardiac care, nutrition, child counselling, teen health and mental health initiatives.
Larose said the group gets referrals from the 36 family doctors across the Georgian Triangle and contacts them within two business days. She said in the past year, more than 2,200 people have been referred.
Larose said the goal of the team is to take the pressure off family doctors and the emergency room. She said a lot of the work they do is preventative.
